Leading Aircraftman Smith was one of four airmen killed in the crash of their Anson (#7566) aircraft during a bombing practice; the crash occurred at Willow Grove, Ontario, 3 miles east of the bombing target, following the mid-air collision with another Anson. Three airmen in the other Anson also perished in this accident.
